Prior Authorizations Procedures or surgeries undertaken as self-pay cash bundled services do not require prior authorization, often allowing for more timely care. At times, our office can arrange a same-day consultation and surgery.

Insurance co-pays and personal responsibility fees assessed by cost sharing programs are due at the time of service.
*Patients with high deductible plans may opt to self-pay if they prefer to benefit from the lower pricing of bundled fees. NOTE: Cash bundled procedures or surgeries are not billed to insurance, therefore, fees will not be applied to a health plan deductible.
Self-Pay Patients: Fees are due by the day of service unless other arrangements have been made. In an effort to ensure that services are accessible to patients regardless of their financial situation, CHS offers patient-friendly payment options that fit most budgets, and patient assistance is available for those who qualify. Medical Expense Sharing Patients: CHS cooperates with all medical expense sharing programs (e.g., Samaritan Ministries, Medi-Share, Christian Healthcare Ministries, Sedera, etc.)
